Minnesota School Immunization Law

Minnesota statutes, Section 121A.15, requires all students enrolled in elementary or secondary school to show evidence of immunizations or documented exemption.

All students in ISD#533 are required to be immunized against communicable diseases including: diphtheria, tetanus, pertussis, measles, mumps, rubella, chickenpox, hepatitis B, polio. New state mandates require that 7th graders receive at least one dose of meningococcal vaccine and a Tdap booster.  Every student needs to have an immunization form on file in the health office, showing evidence of immunization. These requirements can be waived only if medical exemption form (signed by a physician)  or a conscientious objection form (signed by a notary) has been provided. New and current exemption and objection forms will be requested at the kindergarten and 7th grade levels. These forms will not be accepted without the proper signatures.

Students enrolling in ISD #533 for the first time will not be allowed to attend school unless an immunization form is presented to the school office within 10 days of admission.

Students currently attending ISD #533 will have their immunization record reviewed annually for compliance with the Minnesota immunization law for school immunizations.  If the record is not in compliance, the student will be excluded from school until missing immunizations are completed or immunization series started.  Parent/ Guardian will be notified of non-compliance.

 Two doses of chickenpox vaccine is required, but proof of the month/year of chickenpox as a disease will serve as immunity in place of one or both doses.
